During the course of the complaint assessment, Licensor Lauilefue concluded that more likely than not a licensing violation occurred. Licensor Lauilefue based on gathering facts, learned through the complaint assessment that staff were aware of a bruise on the child, but both neglected to report to appropriate channels.
